(1) A VALID PASSPORT.
(2) AN ORIGINAL BIRTH CERTIFICATE
(3) IF APPLICANT HAS BEEN DIVORCED, THE ORIGINAL DIVORCE DECREE.
(4) IF APPLICANT HAS BEEN WIDOWED, THE ORIGINAL MARRIAGE AND DEATH CERTIFICATE OF THE DECEASED SPOUCE.
(5) IF THE APPLICANT IS UNDER 18 YEARS OF AGE, A SWORN CERTIFIED AFFIDAVIT OF CONCENT STAMPED BY A NOTARY PUBLIC.
(6) IF THE DOCUMENTATION IS NOT IN ENGLISH A TRANSLATOR MUST BE PRESENT.
