YOU WILL NEED THE FOLLOWING: 

(1) A Valid Passport, (2) A Birth Certificate, (3) If the applicant has been divorced, an original Divorced Decree, (4) If the applicant has been widowed, the original marriage and death certificates of the deceased spouse, (5) If the applicant is under 18 years of age, a sworn certified affidavit of consent stamped by a Notary Public, and (6) If any document is not in English, a translator must be present.
